What is anesthesia for organ transplantation ?

Anesthesia in organ transplantation provides support for the patient from the preoperative phase through to the first few days after the operation.

In the preoperative phase, the anesthesiologist assesses the patient’s major vital functions (mainly hemodynamic and respiratory) and optimizes the patient’s frailty in collaboration with the organ specialist (anemia, pain, sarcopenia, and capacity for physical effort). We inform the patient about the procedure, answer his questions, and manage anxiety in collaboration with the department’s psychologists.

On the day of the transplant, we welcome the patient to the operating room for a final assessment and then provide anesthesia. For lung, kidney, and uterine transplants, general anesthesia is combined with local-regional anesthesia. General anesthesia consists of :

  • inducing a loss of consciousness known as sedation, using drugs to rest brain activity,
  • ensure the absence of nociception (unconscious pain mechanisms) using morphine-derived drugs and local anesthetics
  • immobilize the patient with reversible drugs to ensure maximum comfort for the surgeon

Anesthesia is adapted throughout the procedure according to the information provided by multimodal monitoring. At the end of the procedure, if the major vital functions are controlled, we stop administering anesthetic drugs to allow the body to eliminate them, and the patient wakes up spontaneously and comfortably.

Loco-regional anesthesia consists of interrupting nerve transmission of nociception (or pain) by administering a local anesthetic in contact with the nerve. The concentration of local anesthetic in contact with the nerve and the characteristics of the nerve (large or small, myelinated or not) will result in the absence of pain, fine sensitivity, or motor skills.

In the postoperative phase, we take care of the patient’s return to normal functioning and comfort. Pain management is an essential part of our job. We monitor the patient’s progress daily, adapting our therapeutic strategy with medication, local anesthesia, and complementary techniques such as TENS (transelectrical nerve stimulation), hypnosis, and auriculotherapy.
